I include a lot of probably unnecessary details, but I did in case any of them can help resolve the issue. I also found a RAM error when running Apple Hardware Test, so I decided to replace it too 2x8 Crucial, 2x2 Hynix, both seemed to be the right specs, so supposedly faulty. I first removed the Fusion Drive CoreStorage Volume with terminal in recovery mode, and then opened it up. I removed the logic board and repasted the CPU. I ran into some installing issues during the migration, which I resolved by canceling, deleting the users, and starting over again. I also ran first aid on both the backup and the computer drives to try to help resolve the issue. I then reinstalled some of my apps. It seemed to be working great, and much faster than before, besides a couple glitches which is expectable of a 9 year old computer. I then booted up Minecraft, which I play occasionally, too see if it would work. And this is where I had my problem.

It now happens a few times a day and can last for 5 mins if I am unlucky. A you can see in the picture the screen splits itself in half horizontally and the the right hand side splits itself in half vertically. All 3 screens show the same thing but color is off and it is all scrunched up.

The glass panel is fixed onto the front bezel with fourteen magnets around its perimeter. To attach the suction cups we sell, first position the suction cup with the movable handle parallel to the face of the glass panel. While lightly holding the suction cup against the glass, raise the movable handle until it is parallel with the other handle. If your suction cups refuse to stick, try cleaning both the glass panel and the suction cup with a mild solvent such as Windex. The glass panel has several positioning pins around its perimeter.
