Karlach is a Tiefling barbarian companion that you can recruit in Baldur's Gate 3. She quickly warmed her way into everyone's hearts, becoming a fan favorite. This makes many of her endings all the more tragic. If you're not yet familiar with the character, she has an infernal engine in her chest where her heart should be, placed there by the arch-demon Zariel. It causes her to burn red-hot, preventing her from touching anyone. How you go about helping her with her problem has a huge bearing on her story ending.

The game puts the player in many difficult situations, with some even testing their morals. One of these situations is a conflict between Wyll, one of their companions, and Karlach, a supposed Devil he is hunting down. To find Karlach, the player must head towards the Risen Road. They can get there after heading north from the Blighted Village and after jumping over a broken bridge where they will have to face hyenas before reaching her. Once the Hyenas are defeated, the player should continue forward, until they find a log that leads to the Forest, where Karlach will be found. The main goal of which includes hunting down Karlach, who he sees as a Devil and a danger. When Wyll finds Karlach, he will want to kill her. However, the player has the option to convince him otherwise as they realize she is not a bad person, and not a Devil, but a Tiefling. If the player is successful, Wyll will soften up and let go of his goal of killing Karlach. In exchange, Karlach joins the player, and she is quite powerful as Barbarian Tiefling. If the player wants, they can instead side with Wyll. This will cause a fight with Karlach, where the player has to kill her. If under-leveled, it can be a tad hard, as she is at level 3 and has 32 hit points. Wyll will not be turned into a Devil, but he will not feel great after the fight. The player also loses a powerful possible companion.
Anders will have something to say about that.

The problem with Karlach and Wyll can all be summed up with one word: Mizora. Mizora is Wyll's warlock patron who has tasked him to find and kill Karlach.

However, you can take a shortcut if you go to the riverbed north of where you found Scratch the Dog. The way I got over to The Risen Road from here is by using my jump mechanic to jump into the middle of the river and I jumped again to get to the other side. If you head south, the Tiefling Karlach will warn you not to come closer. You can make her back down via a skill check with Intimidation or Persuasion. If you agree to help, many of your party members, like Wyll and Astarion approve of this. She also has a tadpole in her head and is romanceable.
