His reasoning for joining was due to feeling indebted to Shintaro Kazama and the Yakuza for paying his ill sister's medical bills. Throughout the events of Yakuza 0, Akira was heavily against Kiryu's approach against the many trials he faced. After Kiryu was framed for murder and resigned from the Dojima Family to protect Shintaro Kazama's reputation, Akira stayed within the Yakuza. Upon Kiryu joining the Dojima Family's rival, Tachibana Real Estate, Akira, along with the others within the Family, were sent under the orders of Hiroki Awano, to hunt Kiryu down and return him to the Family, so that he can be sent to "The Hole" to suffer his punishment. Akira had planned to kill Kiryu to save him from the Dojima Family's torture, but was unable to. Kiryu tells him to act as if he never saw him, and promptly steals his fancy car, returning to Kamurocho to finish what he had started, to Akira's dismay. Kiryu later runs into Akira, who was ordered to kill Kiryu for causing so much trouble for the Family. One of the members offers Akira a promotion if he kills Kiryu, but he refuses and joins Kiryu in battle, later being Kiryu's aid throughout the events of the story. Akira Nishikiyama is a man of medium height and average weight. He has dark shoulder-length hair, which he left down in his younger days. He has diagonal eyebrows and brown eyes.
