By default, when a character asks to use an item, pressing R will deny that request so they don't use the item. If you don't press R, the character will automatically use it after a couple seconds. If you don't like that, you can go into the settings and change it from Deny to Allow. This will make it so pressing R will make them use the item right away instead of having to wait the couple seconds, and not doing anything will deny the request. This is my personal preference.
